Please follow these guidelines when filling out the form. 

*Item Name    ie Day At the Ball Park with Mrs. Jones.  Please enter EXACTLY as you want it to appear in the online auction preview. It will not be edited

*Estimated Value $    This should reflect the value of the donation for a tax receipt/thank you.  If unknown, enter "1".  This will not be displayed in the online auction preview.

*Item Description   Describe the item EXACTLY as you want it to appear online in the auction preview. It will not be edited.  

*Notes   Please enter any information about the donation ie if it will be bundled with another donation from someone else, etc.

*Click "Individual Donor" and "No" for the anonymous question.

*Enter the donor's name and contact info.
