Pay to Play Parties Donation Form The St. Cecilia Mothers Club Spring Fundraiser is an annual fundraising event that supports a variety of school needs such as technology, education and overall school environment. Thank you for making this a success! All donors will be recognized in the event program and the event website. Pay to Play Parties are “sign-up” events that parents or business agree to host for a set number of guests. The hosts cover all expenses of the party, and 100% of the funds raised by the event benefit our school. Please follow these guidelines when filling out the form. *Item Name what your would like to call your party *Estimated Value $ How much will you be spending on the party? We will send you an tax receipt. If unknown, enter "1" *Item Description Please describe the party, including WHO the attendees are (ie adults, families, kids (specify age/grade), women, girls (specify age/grade), men, boys (specify age/grade), other (please describe), WHAT the party will include (food, drinks, entertainment, etc), WHEN, WHERE (if TBD please note) and any other details. Also include the maximum number of participants and suggested cost per person. *Party Hosts Please list first and last names of the party hosts. *Click "Individual Donor" and "No" for the anonymous question. *Enter your name and contact information and submit.
